Sport physicals are required annually for those students participating in extracurricular athletic sports. The exams are valid for 395 days.  Completion of contact information and health history with student/parent signature is required.  Parents are to complete all contact information and health history, with athlete and parent signatures, prior to submission to the school office.

Sport physicals are required every 12 months for students who practice, try out and participate in sports. 

The health care provider may indicate approval for sports on the "School Exam" form.  If your child will be entering 6th or 9th grade next Fall 2019, please ask the health care provider to complete and indicate approval for sports on the “school” exam form. Please note that a physical exam completed on an IHSA/IESA sport physical form DOES NOT meet the requirements for the school exam.
