If you are interested in referring your child (student) for counseling, please complete the referral form and turn in to the office.

Counseling Information and Consent Form

This form will provide you with information about the school counseling program and will allow you to print off a consent form. By signing the consent form,you are giving permission for the school counselor to work with your child.
